The Dame Mary Perkins British Citizen Group - Nomination Form

For six years The British Citizen Youth Honours (BCyA) have been highlighting the incredible work young individuals have been doing throughout Britain, giving back to their communities and making a positive difference to people’s lives. A new category that recognises the contribution made by groups of children – such as those from a community youth club, a classroom or school year, or who attend youth organisations such as the Brownies, Girl Guides, Scouts or any of the other independent youth groups – who has gone above and beyond for their community and society has been introduced. Dame Mary Perkins is Patron of the BCyA and also founder of Specsavers who have agreed to make a donation to the selected Youth Group to help them to continue making a difference in their community. Simply complete this form for your Youth Group to be considered. The successful group’s leader and ten members of the group will be invited to attend The British Citizen Youth Group Medal Presentation Ceremony in October 2024 to receive a bespoke mounted medal and certificate in a display case for the entire group to enjoy.

You are also giving a declaration that the information provided is true to the best of your knowledge. If selected to receive the Dame Mary Perkins British Citizen Youth Group Award, a member of the organisers of the group will be invited with ten of the young members to attend the Presentation Ceremony at the Palace of Westminster in October 2024. Due to the capacity of the venue, the BCyA is unable to increase the number of invitations except in exceptional circumstances. Full joining instructions and official invitations will be sent in advance. The BCyA will provide lunch and light refreshments to guests at the Palace of Westminster before presenting the Medal specially mounted in a display case. (All other costs associated with travel, accommodation, food etc, have to be met by the group or individuals). Following the official presentations, guests will board the BCyA Open Top 'Lap of Honour' Tour taking in some of the sights of the Capital. The youth group selected to receive this award will also be presented with a contribution to the cause courtesy of Specsavers to help them continue their good work.
